The book is organized around four categories of intellectual failure: the foundation of clear thought, the quality of evidence, the direction of attention, and the character of the thinker. The fifth chapter examines the accident at Three Mile Island —a catastrophic event where the disaster stopped—as a combination of all 15 standards under extreme pressure. A Postlude presents an unedited exchange with an AI language model on whether these standards can survive in an information ecosystem increasingly shaped by systems that generate confident answers without knowing whether those answers are true. Peter H. Tyson is a retired U.S. Navy Commander with 21 years of service as a helicopter pilot, developmental test pilot, and Aviation Engineering Duty Officer.
