In November 2020, an AI system called AlphaFold cracked a problem that had stumped biologists for fifty years: predicting the three-dimensional shape of a protein from its genetic sequence alone. That single breakthrough opened the door to the fastest decade of progress the life sciences have ever seen. The Biology Engine is a clear-eyed account of what artificial intelligence has actually delivered in medicine and aging research since then, and what it has not delivered yet. William Liu traces the engine from its first gears to its outer edges. He explains how AlphaFold's database of predicted protein structures grew to cover nearly every known protein, how generative AI began designing molecules that do not exist in nature, and how a handful of AI-discovered drugs moved from target identification to human trials in months instead of years. He goes inside the labs using AI to hunt for new antibiotics as old ones stop working, follows the algorithms now reading medical scans faster and sometimes more accurately than radiologists, and examines the AI-designed cancer vaccines built for a single patient's tumor. The book does not stop at treatment. It follows AI into the harder, slower work of diagnosing rare diseases that took families years to name, reading the ninety-eight percent of the genome that does not code for proteins, and using gene-editing tools guided by machine learning. In its final chapters it turns to aging itself: the biological clocks now used to measure it, the cellular reprogramming experiments trying to reverse it, and the first pills for aging being tested in dogs before they are tested in people. Every figure in this book is sourced to a peer-reviewed paper, a regulator, or a named company disclosure, and the author is careful to separate what AI has proven from what it merely promises. The result is neither a puff piece for Silicon Valley nor a dismissal of real scientific progress. It is an honest map of a fast-moving field, for readers who want to understand what has actually changed in the fight against disease and aging, and what still has not.
